Destroyed by Werewolves at Level 2 during the 'Chasing the Ogre' campaign.
Kama was a ninja who had been lucky enough to avoid meeting the Grim Reaper more than a few times in his short travels. He survived being shot by the party's own archer. He survived being pushed off a high village pallisade. He avoided dying when facing a horde of Gibberilngs. He was accidentally cut deep by the party's swordsman. He survived falling off a tall watchtower. He even survived being impaled by a spike trap in a Valfyrie hideout. His luck clearly wouldn't hold, and Kama finally met his end fighting a bunch of 'The Lightning Wolf’s' henchmen. The battle was going well until a wolfman scored a critical hit on the already injured Kama. One of the other players said “Don’t worry, the DM would need to roll a 5 and a 6 on 2D6 to kill you”. Logic Death intervened, and a 5 and a 6 were actually what I rolled.

Not wanting to pay for his drink got him killed him at Level 1 during the 'Night of the Jackals' campaign.
The party were on a low-key information gathering trip into a town controlled by an enemy Lord. The party Ranger was busy picking a fight with the local guards so Lars slipped away, undetected. Going into a makeshift bar, Lars attempted to bluff a free drink out of the barkeep. This resulted in a chase down the street during which Lars slapped the barkeep and then threw the contents of his stolen drink over the barkeep’s head. The barkeep zonked Lars with a cudgel causing Lars to flee into a pack of waiting guards speared him on their pikes as he tried to run through them. An ignoble, avoidable death resulting from a silly action. Still, Lars was joined in death quite quickly by his equally stupid Ranger friend (Arathorn) who had been trying to bluff the guards into thinking that Lars was an outlaw that he was trying to bring to justice...

Causing trouble in town and assaulting local guards got him killed at level 1 during the 'Night of the Jackal' campaign.
This character died under Bryan's control, but was actually Lindsey's character at the time.
The party was looking to have a low-key information gathering session in a village that was controlled by an enemy Lord. To help their chances they left Arathorn's bear companion outside the village limits and let their most ’local’ looking party members do the talking. While walking through the village, one of the party members was unfairly harassed by a local guard. In a fit of protectiveness, Arathorn went to punch the guard but was prevented by party's Ogre-slayer. All was well with the information gathering mission until the party fighter (Lars) picked a fight with a barkeep further up the road. As the guard that had harassed the party earlier tried to run past Arathorn, the big man clothes-lined the guard, sat over him and started an interrogation. After Lars had been dealt with by the barkeep and some other guards, these guards turned their attention to Arathorn who tried to bluff them into believing that the fighter was an outlaw that he was trying to bring to justice. he was unsuccessful in his bluff and a fight ensued, leading to his death as a trouble-causer, liar and potential threat.
Arathorn's bear companion is all alone, wandering around outside the village. They say that once a year he sits on a village guard in memoriam.

Kobolds got him at level 1 because his party didn't back him up during the "Kobold Hall" adventure.
Playing through a short adventure, clearing out a Kobold encampment underneath a ruined mansion house, Wolfrun led the assault from the front with his Dragonborn friend, shielding the more nervous, less hardy adventurers from the worst of the fighting between them. When the odds got overwhelming, Wolfrun found that the squishy friends he had been protecting were too late to aid him and he was vanquished by a Kobold counter-attack.
Kobold killer killed by Kobolds. A poetic epitath, really.
